Imaging findings
Cardiac MRI showed a large, multi-lobulated, unusual-appearing mass arising from the left atrial appendage region, prolapsing into the left ventricle. Subtle rib lesion and a complex liver lesion were also noted. Post-operative CT showed enlargement of the rib and liver lesions.
Key takeaways
The unusual appearance of the cardiac mass and the presence of synchronous rib and liver lesions suggested a diagnosis other than a mixoma. Pathology confirmed a primary cardiac intimal sarcoma with rhabdomyoblastic differentiation, with the other lesions presumed to be metastases. Primary intimal sarcomas of the heart are extremely rare.
