Imaging findings
Patient with congenital heart disease, d-transposition of the great arteries, status post-arterial switch. The pulmonary artery was draped in front of the aorta. The ventricular morphology was abnormal, with the right ventricle visible, but the left ventricle appearing unusual with a peculiar left ventricular outflow tract. The left ventricle overlies the right ventricle, and morphologic assessment confirmed this: the left ventricle lacked papillary muscle arising from the septum, while the right ventricle showed a septal papillary muscle and moderator band. The left coronary artery descended immediately posterior to the heart in the true anteroposterior dimension.
Key takeaways
Crisscross heart, or superoinferior ventricles, represents an abnormal looping of the ventricles during development, which can be challenging to trace. Distinguishing ventricular morphology by identifying key features like the septal papillary muscles and moderator band is essential for correct characterization. This condition is often associated with other congenital heart diseases.
