Imaging findings
Abnormal echo led to cardiac MRI showing increased soft tissue along the anterior papillary muscle, iso-intense on T1-weighted sequences, with T2 hyperintensity (possibly flow-related). Post-contrast images demonstrated delayed enhancement engulfing the papillary muscle.
Key takeaways
Papillary fibroelastomas, though often thought of as valvular, can occur on any endocardial surface, including papillary muscles, and characteristically demonstrate delayed enhancement due to their fibrous tissue content.
