Imaging findings
CT demonstrated massive dilation of the right heart, with a very dilated right ventricle. The right ventricular free wall appeared almost absent, replaced by pericardium and fat. Fat attenuation was visible within the interventricular septum and in other subtle areas, appearing to invaginate into the myocardium. A thrombus was noted within the right ventricular chamber. Aneurysmal outpouchings were also present in the right ventricular wall.
Key takeaways
Arrhythmogenic Right Ventricular Dysplasia (ARVD) is a condition characterized by progressive fibrofatty replacement of the right ventricular myocardium, leading to RV dilation, wall thinning, and ventricular arrhythmias. Imaging, particularly CT, can reveal hallmark features such as global RV dilation, extensive fat infiltration into the RV wall and septum, and aneurysmal outpouchings. Some consider it part of a spectrum with Uhl's anomaly.
